Disclosure form (50K) - Required form, describing my practice to you. This includes a few words about what to expect in therapy from me, what my rates are, confidentiality, and your legal protection.

Intake Information (48K) - This form is for you to fill out your basic information (name, address, contact info., etc.) and bring to our first session.

MHNet EAP form (154K) - If you are coming to me through your Employee Assistance Program MHNet, this form is required before we can meet. Bring it to your first session.

Staying Happily Married (323K) - A little booklet of tips and ideas about maintaining your happy marriage, or making your marriage happier.
